Do the doctors run on time? Generally the doctors do run on time. Exceptions would be a delivery or emergency. We do try and reach patients to inform them of the delay. You as a patient can help by being on time.
Do we offer discounts to cash paying patients? For office visits a 10% administrative discount is offered when payment is made on the day of service.. The cost of the visit will not be finalized until after the patient has been seen by the doctor. For surgeries a 20% administrative discount is given for cash paying patients. We will require payment of the estimated amount prior to surgery.
Is there a “Release of Medical Records” on line?
Yes, the application is available on line.
There is a$35.00 charge unless the records request comes from a doctor’s office and they are requesting no more than three years worth of records. We will include any surgeries that we have a record of no matter how old. Medical records are not available past seven yrs. of your last date of service.
The standard rate is a $35.00 administrative charge; it could be more based on the size of your chart and whether it is being mailed. If the chart has to be pulled from the “archived records” in storage there will be an additional charge.

What is the turnaround time when I leave a message for the doctor? If you call before 4:00 p.m. Monday thru Friday your call will be returned by the end of the day. Please note, due to HIPAA regulations, a message may not be left.
What is the turnaround time for prescription renewals? Monday thru Friday allow 48hrs.
What labs do we use? We primarily send our patients to Hunter or Quest labs; however you can go to any lab of your choice. It is the patient’s responsibility to go to an “in network” lab provider.
Are you a contracted provider for my insurance? Please look on the Insurance Page for a list of our contracted insurance plans.
What if you are not contracted with my insurance plan? Many patients can see an “out of network” provider. Please see the above payment policy for cash patients.
Why do I still owe money after my co-pay? Usually insurance companies have three levels of patient financial responsibility:
Co-pay
Co-insurance
Deductible. Please verify your benefits with your insurance company.
Are you able to take my payment over the phone? Yes, we accept Visa and MasterCard.
How do I file a disability claim?
State Disability Claim forms: Bring in your completed State Disability claim form no sooner than one week before you intend to be out on disability. The doctor will complete their portion of the claim. Any missing information on your part will delay the filing of the claim.
Private Company Claim Forms: Bring in your completed Private Disability claim form no sooner than one week before you intend to be out on disability. The doctor will complete their portion of the claim. Any missing information on your part will delay the filing of the claim.
There is a $10.00 charge for each different claim package your company may require.
